Welcome! Your first steps to joining Farmington Municipal Schools start here.

Please note, the New Student Enrollment Application is for students who are not currently enrolled in a Farmington Municipal School.

Only a parent with legal custody or a person, who serves as a formal guardian, with a Power of Attorney for educational and medical purposes, may enroll a child in Farmington Municipal Schools.  If you want to enroll in a school outside your attendance zone or you live outside our school district, you will complete the form labeled Waiver.  You will be able to complete the waiver and submit it.  (Please remember that your student will not be enrolled until the waiver has been approved.)

School Age Person is defined as students age 5-21.

Kindergarten Students must be 5 years of age prior to 12:01 a.m. on September 1st of the year of enrollment.

First Grade students must be 6 years of age prior to 12:01 a.m. on September 1st of the year of enrollment.

Special Preschool students must be at least 3 years of age upon entering preschool.

Pre-Kindergarten students must be at least 4 years of age prior to 12:01 a.m. on September 1st of the year of enrollment.

Proof of child’s data of birth

  • Birth certificate (state certified – no copies accepted), or
  • Passport/Visa, or
  • Other legal/notarized identification

Proof of parent/guardian identity

  • Driver’s license, or
  • Passport/Visa, or
  • Motor Vehicle Administration identification card, or
  • Other legal documents used for identification

Proof of parent/guardian relationship or custody

  • Birth certificate which identifies the parents, or
  • Court order, or
  • Separation or divorce decree, or
  • Other legal documents
  • Current immunization record required
  • Transfer documents if applicable

Proof of residency to determine child’s school attendance zone

Proof of residency can include:

Homeowner –

  • Current property tax bill, or
  • Current utility bill (e.g. electric, water, gas, sanitation), or
  • Purchase agreement or
  • Construction contract

Renter -

  • Current rental lease; if lease is over 1 year old, copy of lease and a current utility bill.

Shared Housing –

  • Copy of current property tax bill of homeowner with whom student and parent are living, or
  • Copy of rental lease (if lease is over 1 year old, copy of lease and current utility bill) of renter with whom student and parent are living; and at least two supporting documents.
